Both of you are wrong I think.
For one they were not trying at gore...more of a horrory image. It isn't hard to do something with gore.
As for the needles. It symbolizes the blood drive. the poster is advertising a blood drive. So they added a nurse with tons of needles and the saying ''give until it hurts.''
